Koorliny Moort
Koorliny Moort means 'walking with families' in the language of the Noongar people of the Whadjuk Nation. The Noongar people are the traditional owners of the land on which Perth Children’s Hospital is located.
Who we are
Koorliny Moort is a paediatric service based at Perth Children’s Hospital for Aboriginal children and young people up to 16 years of age.
The service provides culturally appropriate, safe and secure healthcare, supporting families through two key streams.
Services we provide
Paediatric outpatient services
Koorliny Moort provides holistic general and developmental paediatrician clinics for Aboriginal children and adolescents who show evidence of non-engagement in mainstream services.
Paediatric Outpatient clinics run:
- Weekly outreach clinics within the community across the Perth metropolitan area
- Fortnightly clinics at Perth Children’s Hospital.
All Koorliny Moort paediatric clinics are attended by a paediatrician and a nurse.
Community paediatric clinics are delivered in partnership with:
- CAHS Community Health Aboriginal Health Team sites (Bentley, Mirrabooka, Joondalup, Kwinana, Hilton)
- CAMHS sites (Armadale, Clarkson)
- South Coast Babbingur Mia (Rockingham)
- Champion Centre (Seville Grove)
- Cockburn Integrated Health (Success)
- Swan Child and Parent Centre (Koongamia)
- Moorditj Koort Aboriginal Corporation (Midland, Parmelia, Armadale)
Care coordination program
Koorliny Moort provides a nurse-led care coordination program that supports Aboriginal children and their families with navigation, integration, coordination and advocacy across WA Health (tertiary and primary) and community services.
The care coordination program is an outpatient, time-limited service and referrals can be made by a child’s doctor or other health professionals.
Koorliny Moort care coordination includes:
- coordinating PCH appointments to reduce the need for families to travel to PCH
- navigating options for care closer to home or facilitating telehealth appointments with PCH specialities
- supporting shared information across health services including local hospitals, GPs and child health nurses
- collaborating with Aboriginal Medical Services (AMS), Aboriginal Community Controlled Health Services (ACCHS) and other community services to help families meet their health care needs
- assisting families to access the Patient Assisted Transport Scheme (PATS)
- providing health advice and social, cultural and family support
- supporting families at PCH appointments by liaising and linking them with Aboriginal Health Liaison Officers (AHLO’s) and Aboriginal Health Practitioners (AHP)
- assisting with discharge arrangements and follow up appointments.
Eligibility
Paediatric clinics
- Aboriginal children and young people up to 16 years of age
- Reside in the Perth metropolitan area
- Require paediatric assessment for medical or neurodevelopmental concerns
- Are marginalised, at risk and unable to access mainstream paediatric services in the Perth metropolitan region.
Care coordination
- Aboriginal children and young people up to 16 years of age
- Live anywhere within Western Australia
- Have planned hospital appointments at PCH with two or more specialties
- Have complex health care needs
